This is the ".form/dc" template.
It should be called in the following format:

*Labels are defined by page [[Project:.form/dc/menus]].
*Instructions for the dropdown menu itself is the content of a page named, e.g., <span style='color:green'>Project:.form/<span style='color:red'>page-namespace</span>/dc/text</span>. If the form has no custom instructions, then the content of [[Project:.form/dc/text]] is used.
*Templates are named with reference to the namespace of a page being created, e.g., <span style='color:green'>Template:.form/<span style='color:red'>page-namespace</span>/dc:inScheme</span>. Default templates are used if a namespace has no custom template, e.g., [[Template:.form/dc:inScheme]].
*Instructions for a field is the content of a page named, e.g., <span style='color:green'>Project:.form/<span style='color:red'>page-namespace</span>/dc:inScheme</span>. If instructions are not provided by a form, then default instructions are used, e.g., [[Project:.form/dc:inScheme/text]].
